Our Approach to Assessment and Service Planning
360 SUPPORTS LLC’s philosophy of service includes an approach to assessment and service planning. This approach looks at each person’s unique physical, psychological, and social needs and strengths to help define short and long-term goals and objectives. Physical needs are addressed through a focus on the maintenance of basic health (i.e., sleep patterns, nutrition, exercise, personal hygiene) and facilitating access to appropriate healthcare providers for people we support.
Psychological needs are addressed through the provision of counseling and supportive interventions, incorporating awareness of developmental issues, educational interventions to enhance stress management, prevent relapse, and increase overall coping skills and linkage access to professional psychiatric services and facilitation of family involvement, community linkages, as well as the support of peer interactions.
